Özet

In this study, we aimed to compare the anaesthetic and analgesic effects of epidural clonidine and fentanyl. ASA I-III class 44 patients who underwent urological or orthopedic surgery were divided into three groups. In all patients; bupivacaine 0.5% 1 mg/kg was administered epidurally at L2-3 or L3-4 interspace in 0.3 mL/kg 0.9% NaCL (maximum 20 mL) as a volume. In to the local anaesthetic solution in group I; 50 μg (1 mL) of fentanyl was added, in group II; 75 μg (1 mL) of clonidine was added, and the local anaesthetic solution was used alone in group III as a control group. Mean arterial blood pressure, heart rates, respiratory rates, and peripheral arterial oxygen saturations of each patient were noted. Pre and postoperative pain and sedation evaluation was performed with VAS and sedation scale respectively. Maximal block height, onset time to T10 dermatome, degree of motor blockade (with bromage Scale), two segment regression of sensory blockade and its regression to S1-2 segment was evaluated with pin-prick test. In the clonidine group, the onset time of sensory blockade (T10) was shorter and maximal block height (T4) was higher when compared with the other groups (p<0.05). In conclusion, the addition of 75 μg of clonidine to the epidural local anaesthetic was found to be useful because clonidine shortens the onset time of sensory blockade and causes no side effects.
