A prospective study was conducted in patients with undescended testes, to define the frequency of malformation of the epididymis and to investigate its effects on future fertility. Between 1994 and 1996, 104 patients were operated on for 121 undescended testes. During orchiopexy, the tunica vaginalis was opened to search for epididymal malformation. Malformations identified were classified according to the described of Health and coworkers. Eighty percent of our patients were between the ages 3 and 12. The frequency of epididymal malformation was 25 %. The epididymis was completely separate from the testis in 7 patients, only separate from the head of the testis in 8 patients. Long loop vas was found in 2 patients, while one patient had elongation of the epididymis. Seventy five percent of these malformations were found in cryptorchidic testes. Due to the high rate of malformations found in patients with undescended testes, we recommend routine investigation of these structural anomalies, by opening the tunica vaginalis during orhchiopexy.
